Make a leave form: sick / personal / annual
Choose the leave type, enter your name, position and leave dates. The system counts leave in working days (excluding Saturdays, Sundays and the public holidays in the system), then lays it out on the sick leave, maternity leave, personal leave or annual leave form in line with the civil servant leave regulations, with fields for the verifier, the supervisor's opinion and the order.
How to use
- Choose the leave type: sick, personal or annual.
- Enter the place of writing, date, recipient, your name, position and affiliation.
- Pick the first and last day of leave and the system shows the number of working days instantly. (Half-day leave can be chosen for a single day.)
- Enter the number of days already taken this fiscal year (if known), then download the PDF or Word file to sign and submit to your supervisor.
Frequently asked questions
How is leave counted?
Only working days are counted, meaning Saturdays, Sundays and the public holidays in the system are excluded (the same data as the working-day counter tool). Half-day leave counts as 0.5 day. If your organisation counts leave differently, follow your organisation's regulations.
Which fiscal year does the statistics table use?
The government fiscal year runs from 1 October to 30 September of the following year. For example, a leave form dated 17 October 2569 B.E. (2026 C.E.) falls in fiscal year 2570 B.E. (2027 C.E.). The system calculates this from the date the leave form is written.
Does sick leave require a medical certificate?
Under the civil servant leave regulations, sick leave of 30 days or more requires a medical certificate. For leave under 30 days, the approving authority may also order a medical certificate. Private organisations should follow their own work rules and regulations.
Can private companies use this form?
Yes. The rank and affiliation fields can be left blank, and you can use a department or company name instead. The data you enter is used only to generate the file and is not stored.
